It's been 4 years since I saw an OB at North York General (I was seeing Dr. Schilletto) and he did not charge a fee at all. Everything was covered by OHIP and I never paid for anything out of pocket.

I'll be going to York Central, but my OB  (Dr. Benzaquen) just charged me $40 for a form letter (completed and signed by her secretary in about 10 seconds !) to verify my due dates for my mat leave request to my employer.  She has a fee table for non-OHIP covered items, such as duplicate copies of an ultrasound requisition if you lose the first,  or you can opt to pay a $200 flat fee and have unlimited access to those "extras".

My OB at NYGH did, although the fee was optional - he would charge "a la carte", so to speak, if you didn't pay and needed extra services.  I didn't bother and found I didn't need any of those additional services.  And yes, it is totally legit (confirmed, my dad's a doctor too).  There are lots of services that they often need to provide that OHIP does not cover.  My first OB, at Women's College, charged a mandatory fee.  That all being said, if you cannot afford the fee, you can speak to the doctor and s/he is likely to waive it.

To answer the original question, I doubt that all OB charge the fee, and the amount of the fee itself will vary.  It's up to each additional office.

My first Gyn/OB charged me just for a registration package to York Central. I could have gotten it myself. I finally switched to a midwife, and the Gyn/OB charged me $15 a page to fax to the midwife. The midwife told me I don't need to pay for the 3rd fax (its just a form based on my opinion on my history). The Doc's are there to make money I tell you. The service they provide was mediocre and the answers to the 1 or 2 questions I have was unsatisfactory. I refuse to be treated like a lawyer's (liars) client. The midwives answers all my questions and spends an hour approximately with me. Midwives are good if you have a good health history.

It seems like a standard practice for doctors to charge for specific things (ie. notes, etc.).  All my doctors (GPs and OBs alike) charge a yearly fee to cover all incidentals throughout the year or pay as you need them.  We found paying for the year with my dd's paediatrician was worth it's weight in gold.  You have to do what you're comfortable with.  The money covers things that OHIP doesn't. You should get a price list from your OB before you decide if it's worth the money.

For my OB/Gyn there was no such fee 5 years ago, I am very low maintenance in my requirements/demands also. But to charge for registration package is absurd by my standards. My midwife (covered by OHIP also) never charged me for anything.

For my son's pediatrician, there is no fee either. But for the other pediatrician in the same office, I remember there is a sign for fee per service listed (such as phone calls, doctor's notes etc). But if you choose you can also pay the package fee instead (which can be $300).

I've never heard of an OB charging fees. My sister-in-law had a OB at MS and he didn't charge fees then again he wasn't even then during the birth or even afterwards. I had a midwife for my 1st child and now the same midwife for the 2nd we are having in Feb but so far no fees.
